Senior Camp Scholarship Application

The Diabetes Association of Greater Cleveland (DAGC) is offering a $1,000.00 scholarship to a current high school senior who will be graduating in 2008. The applicant must be a former camper who has “graduated” from Camp Ho Mita Koda. The scholarship is to be used towards the camper's college or trade school and will be restricted for that purpose.

The student must complete several easy steps to be qualified for the post high school money, which is outlined below. Scholarship applications must be received by Friday, December 7, 2007 to be considered.

Criteria for selection of the academic scholarship:

The camper must exhibit a desire to continue towards an advanced degree beyond the high school level. He or she should excel academically and show that he/she has put effort to being involved in the community and has been involved in extra curricular activities.

In order to apply, please provide the following:

  • Copy of last two years of school grades
  • Letter of recommendation from a teacher at your present school as well as one additional letter of recommendation from an adult who is not a family member
  • A brief (2 pages or less) letter explaining why you merit the scholarship, what goals you hope to achieve in the future, and what benefits you have derived from your Camp Ho Mita Koda camping experience.
  • Completed Applicant Information Form (46K Acrobat .PDF file)

Please mail your grade, letters of recommendation, essay, and Scholarship Applicant Information form to:

When all of the above are received at the DAGC offices, the camp director will be contacted by the scholarship committee for a reference for the camper.

Applications must be received no later than Friday, December 7, 2007.

Award of the 2008 scholarship will be made at the DAGC Annual Meeting. This meeting will be held after January 1, 2008. The winner and parent(s) or legal guardian(s) will be honored at this event as guests of DAGC.
